Hi all,
I just got back from a trial fitting of Rigid Gas Permeable Contact
lenses and I am sorely bummed! I was in there for about 1 1/2 hours, and tried
on 3 different lenses. They were as (un)comfortable as I expected they would be
for the first fitting, but that was not the real problem. What REALLY bothered
me was that the lenses did not center on my pupil, so that I was constantly
able to see the lens edge; they rode high, up under my upper lid. As I looked
into a mirror, I could clearly see that the len edge was near my (undialated)
pupil, well up into the iris. Additionally, they moved around when I blinked
(which is, I believe, as designed), but they had a ~1 sec time delay for them
to drop back into "position". Before they got back into position, I would see
everything appear to be "smeared out".
What gets me is that the Doctor (Optometry) said that the fit was good and
as it should be. I said that this was NOT going to work for me, and when I
asked what else could be done, he said either
1) a smaller lens so that it would ride betweem the eyelids, or
2) a larger lens so that the edge would be farther from the center.
After I asked him whether anything could be done via a differently shaped
back surface, he said that my eyes had "insufficient toricity" for any help
there (yes, I was a bit PO’ed that he buzzworded me, but I did ask him to
explain the term). I am mostly interested in RGP’s, but I also might (unhappily)
consider soft lenses since their much larger diameter fit beneath the lids.
Oh, when looking straight ahead, my upper lid covers ~1mm of my iris, and
my lower lid is ~2mm below my iris, if this matters.
The lenses I tried were:
1) Boston something or other
2) Polycon 2
3) Polycon 3, I think
Tomorrow morning I am going to hit the Life Sciences Library at UT and see
if I can find any info on just how these things should fit. I already got
a book on "Clinical Fitting of Soft Contact Lenses" (1979, though), and
am hoping for something at a similarly high level for RGP’s.
So, at the end of this rambling, I would ask if anyone could tell me:
1) Is there anything to be done for RGP’s that ride too high?
2) Does this doctor sound like he knows what he’s doing?
Geez, I guess I let my expectations about getting contacts get too high

How much of a factor is pupil size in determining clarity of vision? For
instance, I am nearsighted and I have noticed that the pupil in my right
eye (my weaker eye) is larger than the pupil of my left eye. I have also
noticed that my wife’s pupils (she has perfect vision) are much smaller
than mine in most light conditions. Also, my uncorrected vision is much
better in the day than at night. All of these seem to indicated that
pupil size effects one’s vision. Is enlarged pupil size a cause of or a
condition of myopia? If it is a cause, is it a correctable one?

Does anyone have any experience of PRK at the Optimax clinic in
London (UK). I’m having mine done in 2 weeks. I just wanted to check
(just in case).

I’m trying to find information for my father-in-law. He was recently
diagnosed as having a level 2 macular hole.
He was just examined by a doctor who felt that performing surgery
(Vitrectomy?) could correct his problem. I believe the doctor felt
pretty confident of the operations success too. There are a few
questions my father-in-law has and hopes someone out here could
provide him with some answers.
1) What do you know about Vitrectomies and the chances of this
procedure correcting his macular hole?
2) Any knowledge of the success rate of Vitrectomies?
3) Any recommendations on doctors who could perform this operation,
specifically in the Northeast region of the USA?
Any information will be greatly appreciated as he now feels there may
be some hope but does want to be cautious about going ahead with the
surgery.

Can any kind and informed soul out there explain the
physical ‘why’ of why PRK will not be approved by
the FDA for use in the treatment of astigmatism?
My limited understanding of the physics involved suggests
that it is certainly a possible treatment.
